Dates: April 4-9, 2010
Destination: Nashville, TN
Purpose: We will be working with an organization called The Center for Student Missions. CSM has a very clear mission: to provide students and adults with an effective urban ministry experience that transforms lives, influences local churches and honors God.
Cost: $190
Includes all airfare, transportation, lodging, meals (except travel meals), t-shirt, and all materials. $50 deposit required with application.

STUDENT REQUIREMENTS
1. Have a personal relationship with Jesus Christ.
2. Be a student in grades 6-8.
3. Attend the training meetings beginning the week of February 14, 2010
4. Attend at least 80% of all Sunday morning Bible Study and worship services
beginning February 7, 2010, for the duration of the trip.
5. Attend the mandatory parent/student meeting on Sunday, March 14, at
4:30 p.m. Final payments will be due at this meeting.
REGISTRATION PROCEDURE
1. Submit a completed application with a non-refundable $50 deposit by January
31st.
· Please use blue or black ink—no pencils or gel pens.
· Complete all questions, being as thorough and specific as possible.
· A parent must sign the release form if applicant is under 18.
2. Deposits are refundable only if someone else fills the vacated spot. If the trip
fills up, we will have a waiting list for those interested.
